Any Medi-Cal member in Merced, Monterey, San Benito, or Santa Cruz County with complex health needs, housing support needs, or care coordination needs can receive services at no cost. Once you enroll, you become part of the CalLINKS Community.

No, referrals are not required. You can connect with us directly to become a CalLINKS Member. Providers, hospitals, or community partners may also refer you, but anyone can reach out for support.

No. CalLINKS works alongside your doctors, specialists, and providers. Becoming a CalLINKS Member adds support and coordination, it does not replace your current care team.

Services are provided at no cost to CalLINKS Members. If you are eligible for Medi-Cal, you can receive support as part of the CalLINKS Community.

Yes. Through our Housing Support Services, CalLINKS Members can receive navigation, deposit assistance, and tenancy support to help find, secure, and keep safe, stable housing.
