CalLINKS Members receive personalized support to manage health care, coordinate appointments, and stay on track with health goals. ECM is designed for those with complex needs who benefit from extra guidance, consistent follow-up, and a trusted partner walking with them through their care journey.
Community Supports
Community Supports go beyond traditional health care, helping CalLINKS Members with housing, daily needs, and stability. From housing navigation and deposits to utility setup and connections to local resources, these services address real-life needs so Members can focus on their health and well-being.
Whole-Person Care Coordination
Whole-Person Care Coordination brings doctors, specialists, and service providers together so CalLINKS Members experience seamless, coordinated care. This approach reduces stress, prevents gaps in services, and ensures every Member’s care is centered on their needs and goals.
Health & Wellness Navigation
Health and Wellness Navigation provides CalLINKS Members with guidance and encouragement to access medical care, behavioral health, and social services. Members receive support to keep their benefits in place while staying engaged with personal health goals.
Resource Linkage and Advocacy
CalLINKS Members receive one-on-one help connecting to local programs, resources, and opportunities. We also advocate on behalf of our Members, making sure their voices are heard, barriers are reduced, and every person has the support needed to thrive in the CalLINKS Community.
Who We Are

CalLINKS is a nonprofit organization rooted in equity and whole-person care. Our name, California Linkages, reflects our mission: building the CalLINKS Community by connecting people to the services, support, and opportunities they need to thrive. We work at the intersection of health care, behavioral health, housing, and social services to help CalLINKS Members navigate complex systems with dignity and trust.
We focus especially on underserved communities, including low-income families, immigrants, rural areas, justice-involved individuals, and those impacted by disability or trauma. Our programs are peer-led, culturally responsive, and designed to meet CalLINKS Members where they are walking alongside them toward stability and well-being.

FIRST Steps Together
No family takes the first steps alone
First Steps Together is a family-centered program for CalLINKS Members with children ages 0–5 who are showing early signs of autism or have a new diagnosis. Families are paired with bilingual, culturally responsive mentors who share lived experience. With guidance, emotional support, and practical tools, families gain confidence and connection, ensuring no one takes the first steps alone.

Each leader brings expertise in health care, housing, operations, or advocacy, but more importantly, a shared commitment to walking alongside CalLINKS Members. Together, we strengthen the CalLINKS Community and deliver whole-person care with dignity, respect, and cultural humility.

Founder & Executive Director
Patricia "Trish" Alcocer
Trish Alcocer is a public health leader with over 28 years of experience driving equity, health access, and workforce development throughout California. She has secured millions in grant funding and led major initiatives that transformed how communities receive care in Merced and Monterey Counties.
Trish founded CalLINKS in memory of her brother, Danny Alcocer—whose warmth, humor, and compassion continue to inspire her mission. His legacy reminds her daily that everyone deserves to feel supported, connected, and cared for. Under Trish’s leadership, CalLINKS builds bridges across systems and communities, ensuring no one is ever left behind in their journey toward health and stability.

Co-Founder & Chief of Operations
Sandra Nunez-Rodriquez
Sandra Nunez-Rodriguez, LVN, is a healthcare operations leader with over 25 years of experience advancing case management, compliance, and patient-centered care. She has led multidisciplinary teams across Skilled Nursing Facilities, clinics, and community programs with integrity, compassion, and dedication to equitable service.
Sandra co-founded CalLINKS in memory of her grandmother, Rachel “Mom” Dominguez—whose kindness, generosity, and joy continue to inspire her mission. “Mom’s” unwavering love for others shaped Sandra’s belief that care should always come from the heart. Under Sandra’s leadership, CalLINKS stands as a reflection of that legacy—where professionalism, empathy, and community come together to uplift every person we serve.

Finance Manager
Marisela “Mari” Moreno
Marisela “Mari” Moreno serves as the Finance Manager at CalLINKS, bringing deep expertise in accounting, billing, and program administration. With a career built in both healthcare and nonprofit finance, she ensures every detail—from payroll to claims management—is handled with precision and care.
Mari is known for her reliability, organization, and warm approach to teamwork. A proud wife and mother of two, she enjoys visiting her hometown of Salinas and spending time with family. Passionate about community service, Mari is excited to grow with CalLINKS as it expands across Monterey, San Benito, and Merced Counties, helping more families find stability and hope through equitable, coordinated care.

Lead Case Manager & Program Coordinator
Omar Garcia
Omar Garcia serves as a Lead Case Manager for Enhanced Care Management (ECM) and Community Supports (CS) and coordinates the Early Steps Together program. A lifelong resident of Merced County, Omar draws from his local knowledge and community trust to connect Members with health, housing, and supportive resources. His personal experience as a father to a young son with autism gives him unique empathy for families navigating early diagnoses.
Omar is passionate about empowering parents with education, advocacy, and care coordination that meets them where they are. His dedication ensures that no family feels alone as they take their first steps toward healing, growth, and hope for the future.

Lead Case Manager
Serena Rodriquez
Serena Rodriguez is a Lead Case Manager at CalLINKS whose approach combines empathy, advocacy, and persistence. She develops personalized care plans that address health, housing, and social needs while honoring each Member’s story and background. Serena is known for her strong follow-through and ability to motivate Members toward independence and self-sufficiency.
Her calm yet determined style fosters trust, helping individuals navigate complex systems with confidence. Whether coordinating care or simply listening with compassion, Serena embodies CalLINKS’ belief that small acts of care can create lasting change—and that true healing happens when people feel seen, respected, and supported every step of the way.

Lead Case Manager
Merrian Rodriguez
Merrian Rodriguez brings years of experience from skilled nursing facilities (SNFs) and a natural talent for connecting with people from all walks of life. As a Lead Case Manager at CalLINKS, she uses her extensive network across healthcare sectors to support Members with genuine warmth and understanding.
Merrian is CalLINKS’ top recruiter of Members—people instantly feel comfortable around her and trust her to guide them through challenges. Her strong relationships with hospitals, clinics, and providers make her a vital bridge in care coordination. Merrian’s approachable nature and deep compassion reflect the spirit of CalLINKS: creating community through kindness, connection, and trust.

Case Manager
Joana Ramos
Joana Ramos is a Case Manager at CalLINKS who blends administrative expertise with genuine compassion for the people she serves. Before joining CalLINKS, she worked as an Activities Director in a skilled nursing facility, where she gained hands-on experience in patient engagement, intake, and care coordination. Her ability to build trust and create meaningful connections with residents set the foundation for her success in care management. Fluent in Spanish and deeply familiar with community resources,
Joana approaches every challenge with a “can-do” attitude and calm confidence. She is thoughtful, responsive, and eager to learn—unafraid to ask for guidance while taking initiative to get the job done.

With over 20 years of experience in healthcare, Nicolette Obiacoro brings deep compassion and dedication to her new role as an Enhanced Case Management. Throughout her career, Nico has been a strong advocate for individuals and families navigating complex health and social challenges. Her work spans diverse areas of focus—from supporting elderly populations to empowering at-risk youth—reflecting her commitment to providing equitable, person-centered care.
Nico’s passion lies especially in working with justice-involved individuals and guiding youth toward positive, productive paths. Known for her empathy, advocacy, and unwavering belief in second chances, she strives to make a meaningful difference in every life she touches.
